Make Sure You Have Required Documents

American Audio Visual Center is devoted to working with the best Audiovisual technicians in the industry. Periodically, our work is performed with the aid of independent contractors. If you qualify, we would like to establish you in our Independent Subcontractor Directory.

To comply with the IRS, we need to verify your status as an Independent Contractor. Please provide ALL of the following documents:

1. Certificate of Insurance (Download a Sample), for Workers compensation. The certificate must include; policy number, policy effective date and policy expiration date.

   Complete the Worker’s Compensation Insurance Sole Proprietor Waiver (Click to Download)
2. Business Letterhead
3. Business Card
4. List of 3-5 Clients including Phone Numbers

If you have the documents listed above and have never been employed by AAVC then you qualify to be an Independent Contractor. In this case, you can fill in the website form below.
